Before You Begin
Electronic payment is optional. You are not required to complete the Election Form to receive a payment. If you do not complete it, you will receive your share of the Settlement in the form of a check sent to your last known mailing address.
The deadline to submit your Election Form is December 14, 2026. Please keep a record of your election, including when and how it was submitted.

Electronic Payment
Receive your payment by PayPal or Venmo. You must make your election by December 14, 2026. Your payment will be issued in the manner you prefer after the Court grants final approval of the Settlement.
Paper Check (Default)
If you do not make a timely election, your share of the Settlement will be sent by paper check via First Class Mail to your last known mailing address. If your address has changed, provide your new address on the Election Form or contact the Settlement Administrator.

What is the deadline?
Submit your Election Form online through this website, or by mail, email, or fax, by December 14, 2026 (the Notice Response Deadline). Mailed forms must be postmarked by that date.
Do I have to submit an Election Form?
No. You are not required to complete the Election Form to receive a payment. If you do not complete it, you will receive your share of the Settlement in the form of a check sent to your last known mailing address. If you do choose to submit an Election Form, make sure it is properly completed and timely submitted, or it may delay you receiving your proceeds.
Can I submit my Election Form another way?
Yes. You may sign and date the Election Form included with your Notice and return it to the Settlement Administrator by mail, email, or fax by December 14, 2026. Mailed forms must be postmarked by that date.
